Services Offered

We offer private Occupational Therapy and Case Management services across South Wales and the South West of England. Areas covered include Cardiff, Newport, Bridgend, Swansea, Bristol and Swindon, but we can visit other areas by prior arrangement. We can provide advice on housing adaptations, equipment, and manual handling. We can also complete functional assessments and devise rehabilitation plans to help improve independence with activities.

What is it that you find most important in day to day life? What are the things that you want to be able to achieve? We help you to achieve these goals by considering your needs and helping to reduce the barriers that are preventing you from doing what matters. We can do this by considering alternative ways that the task could be completed to make it easier for example. Or we could recommend alterations to the environment in order to reduce any restrictions.

With 17 years of experience we are able to provide a highly knowledgeable service to best meet your needs. We have extensive experience in housing adaptations, equipment provision and manual handling. We will work with you, your family, and carers to ensure your needs are met as best as possible. Assessments will be in your own home at a time to suit you. Through discussion and observation we can advise you on what we recommend to enable you to live as independently as possible.

Housing Adaptations Advice:

We are able to work with you to assess your needs in relation to any adaptations you may require. You may already have an idea of what it is that you need to make life easier. By requesting our assistance we can help ensure that any adaptations will meet your needs. We will take into account any equipment you use or carers that may need to help you. We can offer a one off assessment to provide advice only in order to help you start your project. If needed we can also be more involved with the planning stages and sourcing of suitable items. We can talk you through the services we provide so that you can make the best choice for you.

Equipment advice:

We can provide an assessment and offer advice in relation to any equipment that may be of benefit. We can also arrange for further assessments with any recommended equipment so you can be sure that it will meet your need before you purchase it. Please note that we are not an equipment provider and you cannot purchase directly from us. We can advise on suppliers and any products we recommend are based on our knowledge and experience in this area. Any information we offer in relation to where equipment can be purchased should not be considered a personal recommendation for the company. You should be sure to research suppliers yourself before purchasing.

Manual Handling Assessments:

If you are struggling to assist someone you care for with getting on and off furniture then we may be able to help. We have extensive experience with manual handling and can assess you to determine how best to help. This may be by offering advice in relation to different ways of supporting the person, or it could be recommending the use of equipment to help both you and them. We can arrange assessments with a variety of manual handling equipment to ensure items are suitable before they are purchased. We can also complete a manual handling plan to share with anyone that helps. This details how best to support the person so that everyone involved is aware of how they should assist.

Functional Assessments:

We are able to assess you in completing a variety of day to day tasks either at home or in the community. If you are finding tasks difficult our input can ensure you are provided with advice to enable you to be as independent as possible. That may be offering advice on how to be more independent with washing and dressing, cooking, laundry, or any hobbies that you may like to do. The assessment process allows us to identify any difficulties and find ways of making things easier for you.

At all times we ensure that your needs are at the centre of what we do. We will always assess you before making any recommendations. Our reports will be clear and concise, detailing the reasons why we have made any particular recommendations.

Case Management

Case Management Society UK state that Case Management is a collaborative process which: assesses, plans, implements, coordinates, monitors and evaluates the options and services required to meet an individuals health, social care, educational and employment needs, using communication and available resources to promote quality cost-effective outcomes.

We have 8 years experience in the Case Management field working with people following minor-moderate injuries. We can carry out telephone or face-to-face assessments. Both will help identify your areas of need and what matters most to you. A rehabilitation plan is compiled based on your goals with a view of helping you achieve greater independence and quality of life. We believe our experience in Occupational Therapy sits hand in hand with the nature of Case Management. We work hard to ensure you are at the centre of the process at all times.
